Such is the cycle of life that with age, comes change. For the ageing population with disabilities, changes can often seem like a daunting experience without the right assistance. Changes can include a medical diagnosis, loss, retirement or relocation. At OPTIMUM CARE, we acknowledge that such significant changes can pose challenges for anyone – and if you or someone you know have a disability, it can prove even more difficult. Hence, our goal is to help our participants receive support in dealing with such adjustments they may experience in their life.

We are NDIS assist life stage, transition registered and approved. This means we can help eligible NDIS participants transition smoothly through life stages by teaching them strategies to gain appropriate skills to deal with such
changes and network their supports.

We offer support with the following services:

• Building a support network
• Support coordination to help use your NDIS plan
• Assistance with accommodation and tenancy obligations
• Life transition planning, including mentoring, peer support and
  individual skill development
• Assistance with improving life choices and decision making
• Daily planning and budgeting
• Managing expenses
• Assessment, training or therapy to help increase your skills and
  independence to participate in community activities
